Registration Checklist

Required Documents for Child Registration

To complete your child’s registration, please prepare the following documents:

Parent/Guardian Information

• Valid photo identification
• Recent pay stubs (6 bi-weekly or 12 weekly)
• Income verification form (to be completed by your employer prior to registration)
• School or training schedule, if applicable
• Proof of address (provide one of the following):
  • Utility bill
  • Lease agreement
  • Letter from landlord
  • Official letter from a government agency confirming your address
  • Parent Affidavit of Residency (if residing in a shared household)

Child’s Information

• Birth certificate for all children under 18 years old
• Immunization records
• Medical card
• Social Security card
